Responsibilities:
- Conduct thorough nursing assessments of patients and document findings
- Communicate and collaborate with physicians and other healthcare team members
- Provide and coordinate patient care with other healthcare team members
- Monitor patients' responses to interventions and report outcomes
- Administer medications as ordered and maintain accurate records
- Provide medical care in the privacy of patients' homes
- Travel to multiple patients' homes daily
- Share on-call responsibilities with a 30-minute response time if required
- Perform other duties as assigned
Qualifications/Requirements
- One year of experience as an RN, with at least six months of recent Home Health experience
- Current RN License
- Current BLS Certification
- Valid, unrestricted driver's license
- Valid auto insurance
- Other certifications or licenses may be required
- Critical thinking, service excellence, and good interpersonal communication skills
- Ability to read and comprehend written instructions
- Ability to follow verbal instructions
