Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless transitions and optimal outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure clear understanding and expectations.
Requirements:
- One to three years experience as a Registered Nurse preferred.
- Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.
- Physical Abilities: Remain in a stationary position, move about, move equipment (50-100lbs), pushing, pulling, bending.
