Under the direction of the Program Coordinator, the Personal Wellness Support Professional works with participants with intellectual and developmental disabilities to ensure that they have support, tools, and resources to lead independent lives.
As a Personal Wellness Support Professional (DSP), you will assist participants with engaging in outings (both local and long distance) and other participant lead activities.
*This is a full-time position with 32-35 hours per week*
As a Direct Support Professional, you will be responsible for:
- Support participants with achieving their personal goals as outlined on their Individual Service Plan (ISP).
- Research, develop, and plan participant focused activities.
- Support participants in the following areas including: independent living skills, social skills, informed decision making skills, and advocacy.
- Assist participants with some aspects of personal care including meal, restroom, and hygiene assistance.
- Regularly support participants with engaging in the community, including traveling on public transportation (fixed route and/or BART) and in the agency vehicle. Community outings will include both local and long distance trips.
- Occassionally support participants with engaging and meaningful on-site activities.
- Maintain detailed and well written case notes using our online platform. Possess good organizational and record-keeping skills
- Protecting the participants' rights and acting as an advocate as needed
- Comply with the Dependent Adult and Elder Abuse Requirements as stipulated by law.
Job Qualifications:
- Strong interpersonal skills and the ability to interact with individuals with disabilities, their family members,coworkers, and supervisors in a positive and constructive manner.
- Positive demeanor, Adaptable, Reliable, and Self Starter.
- High school diploma or GED.
- At least 18 years of age.
- Authorized to work in U.S.
- Helpful but not required. 1 year or more in a Direct Service Provider position including daycare assistance, caregiving, or In-Home Care.
- Must be able to meet Community Care Licensing requirements to pass a Department of Justice criminal background clearance.
- Valid California Driver’s License and reliable transportation. Must be willing to drive agency vehicles both local and long distance to support participants with outings.
- Knowledgeable of computers and software use, MS Office (Word, Excel, Outlook) and G Suite Workplace.
- Good written and verbal communication skills.
- LiveScan criminal record clearance (or exemption) prior to start.
- Health Screening and TB Test clearance prior to start.
- Physical stamina and ability to meet the physical requirements necessary to perform the assigned duties safely and effectively.
Great if you have:
- Knowledgeable of Lanterman Act regulations, Regional Center guidelines, and State of California Licensing regulations.
- Certified in First Aid/CPR/AED by a qualified instructor.
