Title: Case Manager/Individualized Support Provider
Qualifications:
1. Bachelor's Degree preferred, or equivalent experience required with specialization
in psychology, special education, counseling, youth work, or equivalent life
experience
2. Two to five years' experience working with Consumers with developmental
disabilities including experience with behaviorally challenged Consumers
3. Good independent assessment, observation, and organizations skills
4. Excellent computer skills and the ability to operate computer programs (Ex.
Word, Excel, Medisked) and assistive technology devices (Ex. iPad)
5. Good interpersonal skills in a variety of settings and situations.
6. Ability to effectively present oneself, orally and in writing
7. Ability to exercise discretion and work independently with general supervision
8. Reliable transportation; required to travel between multiple offices
Salary: $21.00 - $23.00 hourly
Period of employment: Full-time Hourly (40 hours), non-exempt position
Job Goal: To support the programs of Futures Inc. Consumers. Establishes a
consistent management presence.
ESSENTIAL JOB RESULTS/REPONSIBILITIES:
1. Responsible for opening/closing building or arranging coverage
2. Provides direct service to Consumers in the Middletown area.

3. Implements Behavior Plans & reports accomplishment of goals and objectives
weekly
4. Reports to Supervisor the needs/organizes household materials which may
involve lifting of household items
5. Participates in individual team meetings as needed
6. Develops communication strategies, answers questions and addresses requests
7. Complies with Federal, State, and Local legal requirements by Enforcing
adherence to requirements
8. Represents the organization by Attendance at L10 meetings and participating in
individual team meetings as needed
9. Achieves organization's financial objectives by confirming staff itineraries and
verifying completion of billable hours upon absence of supervisor
10. Contributes to the organization's effectiveness by collecting, updating,
interpreting, reporting and distributing data related to all participants and their
services (Medisked Compliance)
11. Ensuring that issues of performance accountability are routed to the appropriate
supervisor on a timely basis
12. Supervises staff by overseeing/conducting training including orientation for new and
existing staff
13. Maintains professional and technical knowledge by attending educational
workshops; Reviewing professional publications; Establishing personal networks;
Participating in professional societies
14. Identifies issues/trends that require systems improvement
15. Performs office related tasks to ensure operational goals are met
16. Accomplishes related results as needed
