What are the responsibilities and job description for the RN Case Manager position at Gabriel Care?
As a Registered Nurse for Gabriel Care Adult Foster Care, your main responsibility will be to conduct home visits for clients in the community.
The FLEXABILITY of this position allows you to schedule your clients according to what works best for you. Whether you are looking for full-time or part-time hours, we can accommodate your preferences. There is no overtime required, and we provide paid gas mileage for your travel between client visits.
To be eligible for this position, you must have a driver's license, a car, and a cell phone, as driving to clients' homes is a requirement. You will be provided with an iPad and/or laptop to assist you in managing your caseload, scheduling, and completing necessary documentation.
Your caseload will consist of clients who require monthly or bimonthly home visits. The visits will be scheduled by you when it works best for you. The number of clients on your caseload will depend on the number of hours you wish to work.
On average, nurses complete 3 visits per day. To go along with the home visit is a progress note. This form should be completed within 48 hours of the visit to ensure accurate and up-to-date documentation.
The yearly and semiyearly paperwork that is part of the recertification process involves completing a MDS assessment, which is a comprehensive assessment of the client's health status and care needs. This assessment helps determine the level of care and services the client requires. In addition, any necessary PCP order forms should be completed and submitted to ensure that the client's care plan is updated and appropriate.
Overall, while there are some paperwork requirements in this role, they are not overly burdensome.
