POSITION SUMMARY: The Care Coordinator is responsible to Coordinate and maintain scheduling for all clients and caregiver with accurate staffing.
SKILL REQUIREMENTS:

Overview: The Care Coordinator will be required to do the following:
· Handle all Scheduling/Call outs/Rescheduling of clients and staffs effectively and timely. Staff clients’ requests, matching the needs and preferences of the clients with the skills and personalities of the caregiver.
· Monitor missed clock-ins and clock outs and supervise all field operations which includes monitoring employee performance, UpToDate communication with case managers and other relevant stakeholders.
· Report and escalate staffing issues to the Staffing Coordinator and any needed follow ups with case managers, MD, and social workers.
· Handle and resolve all client and staff complaints/concerns and direct urgent and unresolved issues to the QA coordinator to ensure client retention.
· Carry out performance evaluation on all staff (CNAs, LPNs, RNs) twice a year
· Work with the HR coordinator on employee hiring and orientation procedures to company’s standard
· Share on-call duties to resolve evening and weekend issues. Calls must be responded to within 30mins of the call. Please note that good judgement should be applied with emergency cases
· Track and manage caregiver’ availability and overtime status.
· Work with the intake officer on all referrals (Skilled and Unskilled).
· Attend all necessary provider and client meetings as scheduled.
· Assist the administrator/Billing coordinator with billings issues for services rendered
· Research and brainstorm solutions to service problems, complaints, and scheduling gaps.
· Give weekly report to the manager (reports can be emailed). Reports sill include but not limited to the following:
· Ensure 100% client satisfaction
· Assist the company in achieving set goal through marketing for clients
PERFORMANCE STANDARDS
- Accuracy. Ensure accurate staffing of 95%, i.e. no unstaffed hours of over 5% of the total hours on case load
- Maintain an updated list of active and inactive client and employees.
- Ensure to achieve a compatibility rate of 90% in all client and employee matching scheme assists other employees in the work unit in accomplishing assignments.
- Adheres to FSA’s policies and procedures as well as other necessary company program policies and procedures on duties related to this office to ensure compliance.
- Staffing Issues/queries resolved in a timely manner-within a week
- Attends case conferences, team meeting, and education in-services as required.
- Carry out on-site visitation as needed
- Professionalism, punctuality, flexibility and reliability are imperative
- Maintains and ensures all deadlines are met. Must be flexible and display initiative in working on new tasks.
Job Type: Full-time
Pay: $17.00 - $18.00 per hour
