Job Summary
We are seeking a dedicated and compassionate Qualified Professional to join our team in providing exceptional care to residents in our assisted living and memory care facilities. The ideal candidate will have a strong background in home community care, long-term care, and a commitment to upholding resident rights while ensuring a safe and supportive environment.
Duties:
a. Overseeing the services and supports provided to participant for general guidance to the provider agency in areas of compliance and quality improvement;
b. Assuring that the supports provided are within the scope of the agency’s service enrollment and experience to assure effective delivery;
c. Assuring that the services address the participant’s needs and adhere to the application of person centered values, choice and participant’s rights;
d. Providing, arranging or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goal(s) and objectives;
e. Recommend other needed services/supports or changes to the delivery model using a continuous quality improvement approach;
f. Providing consultation to the provider agency in ISP implementation strategies that are specific, measurable, achievable, relevant, realistic and time limited in order to meet the needs and personal goal(s) of the participant;
g. Assess areas of risks either individually or overall risks to persons supported through agency practice, policy or lack of policy or procedures/protocols. Providing risk mitigation strategies to the provider agency;
h. Reviewing that functional assessments are in place to support formulation of the participant’s plan for delivery of all waiver services that include: i. The Health Risk Screening Tool; ii. The Supports Intensity Scale; iii. Functional Behavioral Evaluation; iv. Others (E.g., Nursing, OT, PT etc.) as needed or required.
i. Oversee high intensity services if applicable that address health and safety risks for the participant’s that includes: i. The implementation and effectiveness of Behavior Support Plans; ii. The implementation and effectiveness of the Participant’s Crisis Plan; and iii. Identifying ongoing supports as needed (medical and /or behavioral) in collaboration with agency staff, staff of other agencies providing supports to the participants mutually served or other members of the healthcare team.
Experience
- Previous experience in group home, home community care, or long-term care settings is preferred.
- Knowledge of resident rights and the importance of maintaining dignity and respect for all individuals.
- Certification in CPR is highly desirable.
- Familiarity with group home settings is a plus.
- Strong communication skills, both verbal and written, to effectively interact with residents, families, and team members.
- A compassionate approach to caregiving with a focus on enhancing the quality of life for all residents.
MINIMUM QUALIFICATIONS
§ Master's degree minimum
§ Minimum 16 hours of yearly training
§ CPR/First Aid certified
§ Must be able to read and write English
§ Excellent interpersonal skills
§ Minimum degree in related field as based on DBHDD standards
§ Clean driving record
§ Clean Criminal Record Check
Must have a minimum of 2 years' experience and a Bachelor's Degree in Special Education, Social Work, Psychology, developmental disabilities, Nursing, or related Human Services field.
