A Medical Scribe is essentially a personal assistant to the physician; performing documentation in the electronic medical chart, gathering information for the patient's visit, and partnering with the physician to deliver efficient patient care, thus allowing the physician to focus more directly on clinical care. The scribe observes the physician during all patient encounters and performs documentation on the physician's behalf. Under the direction of the physician, the scribe enters information into the patient's electronic medical record or, at times, written chart. At the physician's request, the scribe must be able to locate all past medical records, prior lab/radiology results, or past visit histories for the physician to review. Additionally, within the electronic medical record itself, the scribe needs the ability to enter data into the chart on the physician's behalf, efficiently and correctly. All scribe-entered data is reviewed and authenticated by the physician. Emergency physicians work any day of the week, all hours of the day or night. A scribe is required to work when the physician works, so open availability is required. The position requires availability at more than one location during a scheduled work week, with some required travel.
- Must be able to commit to at least one year of work with the company. This is not a summer or short-term position.
- Must be available to work at least 10 shifts a month, (2 shifts a week) including two weekends a month.
- Must be willing to have and pass a background check and drug test.
- Must be willing to travel between Gainesville, Newberry, and Starke locations.
The Medical Scribe responsibilities include;
1. Accurately and thoroughly document medical visits and procedures as they are being performed by the physician, including but not limited to:
- Accompany the physician to the examination room in order to transcribe the patient’s medical, surgical, family, and social histories, patient’s ROS, and physical exam. Although physician preferences will be provided to you, you are required to learn these for each physician.
- Procedures and treatments performed by healthcare professionals, including physicians, nurse practitioners, physician assistants, residents, and nurses.
-List all proper diagnoses and symptoms as well as follow-up instructions and prescriptions as dictated by the physician.
- Patient orders such as laboratory tests, radiology tests, medications, etc.
2. Dictation/faxing/phone calls and clerical tasks. These tasks can be asked of a Scribe as directed by the physician, via dictation or summary of the medical record. You may be asked to research contact information for primary care or referring physicians, prepare reports, make phone calls, and other clerical tasks as assigned.
3. Scribes also spot mistakes or inconsistencies in medical documentation and check to correct the information in order to reduce errors. Scribes proofread and edit all the physician’s medical documents for accuracy, spelling, punctuation, and grammar. All charting and documentation must be signed off by a physician. Scribes ensure that all clinical data, lab or other test results, and the interpretation of the results by the physician are recorded accurately in the medical record. Alert physician when the chart is incomplete. Scribes must comply with specific standards that apply to the style of the physician and to legal and ethical requirements for preparing medical documents and for keeping patient information confidential.
4. Attend trainings and/or staff meetings on information such as; technology, legal, HIPAA and regulatory compliance, billing and coding, and company happenings and changes.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
1. Education: High school diploma or equivalent. Must have taken a Medical Terminology class or have at least one year of experience in a similar job field.
2. Language Skills: The ability to write routine reports and correspondence. Scribes must be proficient in typing and spelling, punctuation, grammar, and oral communication. Must be able to listen to complex medical information and summarize in a clear, complete, and concise manner. Excellent English composition skills are required to generate professional and accurate writing. Handwriting must be clear and legible.
3. Understanding of medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments to the extent required to understand and accurately transcribe in the medical chart. Translate medical abbreviations into their expanded forms.
4. Mathematical Skills: Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals. Have the ability to compute ratio and percent.
5. Reasoning Ability: Ability to apply common sense understanding to carry out instructions given in written, oral, or diagram form. Ability to apply logic and draw conclusions based on knowledge. Have the ability to refer to reference materials as needed.
6. Computer Skills: To perform this job successfully, an individual should be able to learn and use all functions of electronic medical record software. Must accurately enter data into the database, search for information, and send and receive email. Must be proficient in Microsoft Word and Microsoft Excel. Must be willing to learn to use other programs such as Google Docs and One Drive. Must have access to and be able to use the internet to access schedules, e-mail, research information, etc. Must be proficient in typing at approximately 60 words per minute.
7. Physical Requirements: While performing the duties of this job the Scribe is regularly required to stand, sit, walk, use hands to type and write with a pen, reach with hands and arms and talk or hear. The Scribe must be able to lift and /or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus on computer screens and printed sources.

POSITION TYPE: Part-Time, typically 2-3 shifts per week and at least one weekend a month. The average shift is between 6-10 hours, up to 12 hours. Hours will vary depending on location and physician.
