Position Overview
The Personal Health Assistant (PHA) plays an essential role in ensuring the overall well-being of clients by providing direct health monitoring, personal care assistance, and lifestyle support. Working under supervision or independently (depending on the client and care plan), the PHA ensures that clients maintain the highest possible quality of life, comfort, and dignity in their homes or care environments.
Key Responsibilities:
- Assist clients with daily personal care, including bathing, dressing, grooming, and hygiene.
- Support vital signs monitoring and health tracking (e.g., blood pressure, glucose levels, temperature) as directed by a care plan.
- Aid with medication reminders and coordination of prescribed treatments.
- Provide nutritional support, including meal prep, feeding assistance, and tracking dietary needs.
- Assist with mobility and transfers, including use of wheelchairs, walkers, or other mobility aids.
- Offer companionship and emotional support to clients coping with health issues, aging, or disability.
- Maintain clear documentation of care provided and report changes in physical or mental health to supervisors or family members.
- Ensure client environment is clean and safe through light housekeeping and organizing personal spaces.
- Accompany clients to appointments, errands, or social outings as needed.
- Collaborate with care teams, including nurses, therapists, and family members, to maintain a comprehensive care approach.
Qualifications:
- High school diploma or GED required; CNA, HHA, or other health-related certification preferred.
- Minimum of 6 months to 1 year experience in a personal care or healthcare support role.
- Ability to monitor and report health observations accurately.
- CPR and First Aid certification (or willingness to obtain upon hire).
- Excellent communication and interpersonal skills.
- A genuine passion for helping individuals live independently and with dignity.
- Must pass a background check and provide up-to-date physical and TB test results.

Pay Rate:
- $15.00 – $19.00 per hour (depending on experience and client needs)
- Expected Hours: 20 – 60 hours per week
