- Key Responsibilities :
- Provide direct patient care, including specialized end-of-life care.
- Implement and follow comprehensive care plans, including accurate and timely documentation.
- Educate patients and families on self-care and treatment plans.
- Coordinate hospice services with an interdisciplinary team to ensure quality patient outcomes.
- Manage pain and symptoms, ensuring comfort and dignity for patients.
- Participate in professional growth and quality improvement activities.
- Requirements :
- Graduate of an accredited nursing program.
- Minimum 1 year of nursing experience (community health / hospice or med / surg preferred).
- Active LPN / LVN license in the state.
- CPR certified and up-to-date on licensure.
- Strong communication and organizational skills.
- Valid driver’s license with a reliable and insured vehicle.
- Passion for working with terminally ill patients and understanding of hospice philosophy.
