What are the responsibilities and job description for the Public Health Home Visiting Case Manager position at Gallatin County?
This position is located in the Gallatin City-County Health Department and performs duties that support the vision, mission and goals of the Department. This position is grant funded and is contingent on future funding. This position is responsible for providing a variety of professional and administrative services within the health department to promote and maintain individual, group, and community health. This position will work with the support of a team of professionals/nurses to oversee the Public Health Home Visiting program and services by providing case management and referral coordination to families to: assess health issues; provide education for healthy pregnancies; provide postnatal education; and complete required documentation. Works with community health care professionals, families, partner organizations, and others to ensure overall well-being of clients; and provides referrals to community resources; and represents clients as an advocate with other social service agencies. Other duties may include work in a variety of disease prevention and health promotion programs, as well as outreach to the public and contributing to departmental response to public health emergencies. This position reports to the Maternal Child Health Services Group Manager and does not supervise other County personnel.
This is a grant funded position, part time (0.30 FTE) or 12 hours per week.
Each duty checked below makes up at least 20% of the job, and all other functions are included in other Duties as Assigned. All duties below are essential functions unless otherwise indicated.
Case Management
- Provides family support to families in the County referred to the Public Health Home Visiting Program in collaboration with the Public Health Nurses and Maternal Child Health Home Visitors. The position provides a variety of professional consultation, referral, education, and support services.
- Serve as a liaison between clients, healthcare providers, and social service organizations to ensure seamless access to medical, behavioral health, and social support services.
- Assesses family needs based on historical and current information.
- Ensure that referrals and services are culturally appropriate, accessible, and inclusive of diverse populations, fostering trust and engagement with clients from various backgrounds.
- Ensures all paperwork is completed, documentation is submitted and all necessary family information is gathered.
- Maintain accurate and confidential records of referrals, services provided, and outcomes.
- Follows up with clients to ensure appropriate services have been obtained and to provide additional education and support.
- Serves as a liaison between the Health Department and other community service programs and providers.
- Responds to crises on an as-needed basis by assisting family members in resolving problems and de-escalating critical situations and events.
Referral Coordination
- Efficiently manage and process referrals for home visiting services, ensuring timely connection between clients and appropriate healthcare providers or community resources.
- Act as an advocate for clients by providing guidance, resources, and information about available services, helping them navigate the healthcare system and overcome barriers to care.
- Contribute to program evaluation efforts by tracking referral trends and client progress.
- Identify areas for improvement in the referral process and suggest strategies for enhancing efficiency, communication, and client satisfaction within the maternal and child health program.
- Serves as a liaison between the Health Department and other community service programs and providers.
- Completes required documentation in an accurate and timely manner for the purpose of keeping current and progressive data related to needs, efforts, progress, outcome, and communication with family.
Community Health
- Enhances community health through participation in a wide variety of local and countywide activities that promote partnerships, collaboration and education associated with effective public health resources and services.
- Collaborates with other organizations and agencies to provide comprehensive health services to individual clients, as well as develop population-based services.
- Participates on community boards and committees as a professional.
Other Duties as Assigned
Perform a variety of other duties as assigned by supervisor. This includes coordinating special projects and events, system and grant research and analysis, representing the profession and the County at meetings, conferences, and special events, attending continuing education and training as required and accounting of time electronically at the end of the workweek. If the County Commission proclaims a local emergency due to an actual or threatened disaster such as an earthquake, fire, riot, flood, etc., County employees may be required to provide services during the emergency and for a subsequent period of assessment and recovery.
This work requires knowledge of the principles and practices, theories and concepts of the field of social work, human behavior and child development, mental health, available community resources and services, case management; record keeping and understand the philosophy of family support. The incumbent is required to have knowledge of parenting education; crisis intervention strategies; education methods and techniques and the working knowledge of healthy parent child interaction and child development and be able to demonstrate skill in utilizing this knowledge. This person must have good problem-solving skills; be able to make independent determinations and have the ability to recognize the need for professional intervention; make appropriate recommendations to the team and work as part of a team. In addition, this person must be able to effectively communicate both verbally and in writing, basic computer skills are also needed. The incumbent must have knowledge of all of the Department’s programs in order to provide assistance as needed. Must possess self-initiative but be able to work as part of a team. Position also requires customer service standards with a high-level of skill in dealing with the public, sometimes under stressful situations.
This position requires knowledge, skills, and abilities that are typically acquired through a combination of education and experience equivalent to graduation from an accredited college or university with advanced course work in social work, public health, or other social services or equivalent combination of education, training and/or experience. Experience in case management or social services, public health or a related area and basic Spanish language preferred.
Licenses and Certifications: Applicants for this position must possess a valid driver’s license and be insurable under the County’s liability policies.
