What are the responsibilities and job description for the Case Manager position at Gandara Mental Health Center, Inc.?
- Provide assessment and service planning to address OUD and identified social determinates of health
- Develop, implement, and monitor service plan consistent with identified needs
- Communicate effective and in a timely manner with persons served and collaterals
- Attend external and internal meetings relevant to individual served and/or services
- Ensure that persons served progress notes, service plans, and other required documentation is completed professionally and accurately and in the electronic health record in a timely manner
- Ensure that consultation on the person’s served behalf is conducted
- Provide wraparound care and serve as primary point of contact for individual served
- Attend relevant trainings as required by Program Director
Education
Preferred- Bachelors or better in Human Services or related field
Licenses & Certifications
Preferred- Peer Specialist Cert.
