Job Summary:
We searching for a Clinical Triage Screener who will ensure that youth referred for Intensive Care Coordination (ICC) and In-Home Therapy (IHT) services receive timely access to medically necessary services in a timely and consistent fashion, referrals for these services will be screened for medical necessity and urgency. The Clinical Triage Screener will hold a Master’s degree in a clinical appropriate field as determined by the managed care entities (MCEs) approving the services. The Triage Screener will be assigned to a Region within the organization.
Duties and Responsibilities:
- Follow all Triage policies and procedures for the CBYCS programs they are responsible for.
- As needed, the Triage Screener will follow all state mandated reporting requirements and emergency protocols.
- Communicate with families in their preferred language and/or find an interpreter if they are unable to speak the language.
- Manage the Triage Waitlist Log for all youth referred for IHT and ICC services when appropriate.
- Screen and Triage all referrals, considering a number of critical factors including appropriateness family choice, urgency (including risk factors), impending events and factors affecting engagement, current services received by youth, other providers for whom the youth is being referred, critical factors that may need to be escalated to the Program Director for review.
- Prioritize waitlists as needed and according to the greatest needs.
- Will conduct waitlist follow up with clients and families, per program specs and guidelines, to determine continued appropriateness to wait and level of care.
- Consistent communication with Program Directors and intake coordinators regarding wait times and case assignment.
- Communication with MCE’s if a youth is waiting for 14 days in order to agree upon a protocol and ensure that the protocol is followed.
- Communication with other external services to determine program vacancies.
- Provide reports as may be required to MCE’s, Gandara Administration, and Quality Assurance.
- Use of MABHA to locate and connect families to the appropriate services and agencies.
- Collaborates with other necessary individuals the youth and family may have contact with, such as Bureau Case Managers, Probation Officers, Judges, District Attorneys, Attorneys, Teachers, Physicians, etc. Will contact these key people by telephone frequently, invite them to Care Planning Team and ICP meetings, and provide them with copies of the ICP.
- Master level clinicians must inform the Agency of all emergencies or critical incidents.
- Will manage the CBYCS waitlist and market services as needed.
- Will provide as needed support to the office manager as needed. Covering clerical and office management tasks.
Minimum Qualifications:
- Experience working within the child/family-serving systems as a direct care worker.
- Experience navigating any of the child/family-serving systems and advocating for family members who are involved in the behavioral health system
- Demonstrated ability to be innovative, creative, analytical and decisive in problem solving.
- Demonstrated communications, administrative and organizational capabilities.
- Excellent Microsoft office skills.
- Must have a driver’s license and availability of vehicle to support Program needs.
- Bi-Lingual Spanish/English a must. Portuguese/Cap Verdean preferable
