To maintain / coordinate potential participant referrals on program spreadsheet and StateTo screen potential clients guided by program admission criteria and ASAM criteriaTo conduct the interview for admission to the RRS treatment programTo accurately update program census sheetsTo review Resident Handbook to new resident upon admission to the house and coordinate an orientation date / time with House Manager assigned that task.Share all expectations of program Direct Care I / RSS position.Duties and Responsibilities :
To attend scheduled staff meetings.To screen all incoming calls, determine their nature, and forward information through proper channels.To maintain client records in accordance with standard format and timetables.To attend scheduled supervisory meetings with Program Director / Clinical SupervisorTo maintain assigned financial record keeping, and submits required reports accurately and in a timely manner.To attend all trainings as required by funding source and the AgencyTo organize residents for Food Stamp withdrawalTo become approved for BSAS EIM / ESM data entryTo input new resident into BSAS EIM / ESM electronic databaseTo input new resident information into Gandara EHR and activate recordTo accurately maintain and update program resident censusTo outreach to potential residents and referral source for referral follow-upCommunity liaison; responsible for attending community meetingsHolyoke safe neighborhood initiative, Springfield’ New North Citizens Council / C3 Community Policing meeting both meetings will connect said programs to resources, keep us in the know of what is going on around the community as in activities and safety issues concerns, and other recovery resourcesResponsible for requesting and confirming PCP / BH provider resident documentation this will aid us in complying with insurance request of physicals and in obtaining BH / MH DX on residents who do not have a therapist out of Gandara.Minimum Qualifications :
