Responsibilities:
- Provide case management services to assigned residents, including advocacy and support.
- Develop and maintain relationships with clients, providing guidance and support as needed.
- Complete all clinical documentation and casework, including assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, and transition/discharge/aftercare planning.
- Maintain updated clinical notes and reporting in accordance with established record keeping procedures.
