Job Title : Admission Nurse LVN Location : Garden City Healthcare Center - Skilled Nursing Facility, Modesto, CA Compensation : Starting at $30 per Hour Schedule : Full-Time (12 : 00 PM - 8 : 30 PM) Job Summary : Garden City Healthcare Center is seeking an experienced and compassionate Admission Nurse LVN to join our dedicated team.

In this role, you will be responsible for managing the admissions process for new residents, ensuring a smooth transition into our skilled nursing facility.

Your expertise in nursing assessments, documentation, and resident education will play a key role in delivering high-quality care from the moment our residents arrive. Key Responsibilities :

• Resident Admissions : Conduct thorough initial nursing assessments for prospective residents to determine care needs and ensure appropriate placement. Collaborate with interdisciplinary teams, including physicians, RNs, and other healthcare professionals, to develop personalized care plans.
• Documentation & Compliance : Accurately document all admission-related evaluations, resident histories, and care plans in compliance with facility policies and regulatory guidelines. Maintain current and organized records to ensure a seamless transition from admission through ongoing care.
• Resident & Family Education : Provide clear, compassionate communication to residents and their families about the facility's services, policies, and care processes. Answer questions and address concerns to ensure a positive experience during the admission process.
• Coordination & Communication : Serve as a key liaison between residents, families, and the healthcare team to ensure comprehensive, coordinated care. Facilitate communication during the transition process to promote continuity of care and optimal resident outcomes.
• Quality Improvement : Identify opportunities to improve admission procedures and enhance resident satisfaction. Participate in ongoing training and quality improvement initiatives to uphold the highest standards of care. Qualifications :
• Licensure & Certification : Must hold a valid California LVN (Licensed Vocational Nurse) license. Current Basic Life Support (BLS) certification is required.
• Experience : Prior experience in a skilled nursing facility or similar healthcare setting is preferred. Experience with resident admissions or transitional care is an asset.
• Skills & Attributes : Strong clinical assessment and documentation skills. Excellent interpersonal and communication abilities, with a compassionate approach to resident care. Detail-oriented, organized, and able to work effectively in a fast-paced, team-oriented environment. Proactive problem solver committed to continuous improvement in resident care processes. Working Conditions :
• Full-time shift from 12 : 00 PM to 8 : 30 PM.
