ESSENTIAL FUNCTIONS:
Responsible for completing the enrollment process of children and families enrolled into the center-based Prevention Initiative program. Works with parents to complete family action plans as required in the Prevention Initiative Program Manual.
Main Job Tasks and Responsibilities:
- Responsible for networking in the community to recruit children and families for participation in the Prevention Initiative program.
- Conducts the enrollment process that determine eligible of infants, toddlers and families to receive Prevention Initiative program services.
- Works with staff to track the follow-up on screenings (developmental social and emotional) and assists with obtaining parental consent for possible needed mental health services.
- Follows established referral procedures, documents and follows up on all referrals made on behalf of children and families.
- Participates in specialized meetings to discuss concerns of children exhibiting concerns with parents to seek authorization of individualized observations to be conducted by licensed mental health professionals.
- Participates in center meetings, agency meetings, parent meetings and case coordination meetings as required.
- Participates in community outreach activities that involve networking to access resources needed for children and families.
- Monitors families progress toward completing the established Family Action Plan as part of the Family Partnership Agreement.
- Reports any challenges with families, crisis or emergency situations, including incidents of suspected abuse, or neglect which is reported to the immediate supervisor and appropriate State agency as required per mandated reporter requirements and regulations.
- Completes monthly reports by maintaining accurate family tracking system and documentation as required to related to family needs, absences and other information required by the funding source. Utilizes procedures for confidentiality in the record keeping system and when obtaining information from other agencies.
- Maintains all forms required in the file system, including case notes, referral forms, attendance information, information related to health or disabling conditions, health information, and any other information required to be in the required for the child and family file.
- Participates in weekly reflective supervision meetings conducted by the supervisor.
- Attends meeting sponsored by the Illinois State Board of Education to obtain knowledge regarding the implementation of the Prevention Initiative Program.
- Perform other duties as assigned by the immediate supervisor.
Requirements
QUALIFICATIONS:
Experience: Bachelor of Arts in Social Work, Psychology, Human Services, Applied Behavioral Science or Sociology with 3 to 5 years of case management experience working in social service agencies.
Credential:
· Bachelor’s degree in Social Work, Psychology, Human Services, Applied Behavioral Science or Sociology.
Required Knowledge, Skills and Abilities
- Ability to communicate and interpret State of Illinois requirements of the Prevention Initiative program.
- Mathematics and language arts proficiency
- Knowledge of Microsoft Office Suite (Excel, Word, Access, etc.), Google Sheets and Google Forms
- Proficient in data entry and management
- Excellent communication and organizational skills
- Able to work collaboratively with teams
- Demonstrated reliability regarding attendance and work performance
- Ability to be creative, flexible and adaptive in ALL activities and responsibilities
