ESSENTIAL FUNCTIONS

Conduct clinical assessments, develop treatment plans and provide individual, family and group therapy to agency clients. Serve as a clinical advocate for clients.  Document and report progress in treatment.

ORGANIZING AND COORDINATING

            Screening/Intake

  1. Assist with the retrieval of necessary documentation for participant enrollment into the program.
  2. Attend participant orientations to familiarize themselves with participants’ interaction with family members.
  3. Develop goals and treatment plans with the individual participant and guardians.
  4. Meet with the participants on a weekly basis regularly.
  5. Establish good communication skills with the supervisor.
  6. Communicate with parents/guardians regularly.
  7. Demonstrate the ability to document clinical notes/reports.
  8. Must have a master degree in counseling/psychology and has LCP license.
  9. Clean DCFS background check.

Interfacing

  1. Ability to communicate and work with children with varying ages and skills; and provide necessary guidance, support, and supervision to participants at all times.
  2. Ability to observe participant behavior and participation, assess appropriateness of such, and enforce safety regulations and emergency procedures.
  3. Maintain positive clinical relationships with clients and their caregivers
  4. Facilitate interaction between participant, and participant as well as participant, and staff.
  5. Participate in department and company employee meetings.
  6. Provide frequent feedback and positive reinforcement to participants

Monitoring

  1.   Use pre assessment and on ongoing assessment tools to monitor the progress and goals that were instituted for each client.
  2.   Monitor and maintain individual participant clinical files and report needed adjustments to Supervisor
  3.   Monitor client progress of each week and document progress on progress notes
  4.   Revaluate treatment process each month and report any findings to supervisor.

Direct Service

  1.   Develop and implement treatments plans for each client.
  2.   Prepare and forward reports to clinical director.
  3.   Build a therapeutic relationship with client.
  4.   Effectively complete client clinical progress notes and submit them in a timely fashion.
  5.   Attend program meetings.
  6.   Available to attend required additional trainings and staffing.
  7.   Provide15-20 hours of direct client contact services per week (in-home and in-office).
  8.   Provide clinical counseling and other clinically related services to a client caseload of 15.
  9.   Conduct clinical assessments
  10.   Develop clinically appropriate treatment plans and goals based on client needs
  11.   Provide linkage to auxiliary clinical services and monitor thru follow-up
  12.   Facilitate/Conduct in-house clinical staffing.
  13.   Attend all meetings surrounding clinical matters including but not limited to PRT’s, psychiatric hospitalization staffing.
  14.   Complete all paperwork surrounding Psychiatric Hospitalization and provide and monitor follow-up care
  15.   Write thorough clinical progress notes
  16.   Provide and/or assist with providing clinical trainings
  17.   Ensure all HIPAA Policy and Standards are implemented and adhered to
  18.   Familiar with DSM IV
  19.   Must be able to conduct and draft mental health assessments
  20.   All other duties as assigned.
  21.   Assist with conducting comprehensive intake assessments
  22.   Co-facilitate intake interviews
  23.   Complete Risk Management and Wrap Plans according to intake assessment and client interview
  24.   Attend intake clinical staffing.
  25.   Participate in participant 90-day Reviews and Discharge Plan Meetings
  26.   Facilitate client related life skills trainings
  27.   Conduct support groups.
  28.   May be required to transport clients.
  29.   All other duties as assigned.

 Documentation/Record Keeping

  1. Prepare and progress reports to the supervisor and quarterly reports.
  2. Documentation of any Unusual Incident of any participants on a UIR Form
  3. Maintain accurate individual participant files and prepare case notes

Agency Activities/Trainings/Other

  1. Attend and participate in all initial and on going training
  2. Actively participate in orientation and staff training, assigned staff meetings
  3. Attend “All Staff” meetings
  4. Attend trainings that offer CEUs.  Must have 10 CEU for the year.


QUALIFICATIONS

Education: Candidates will have a Masters Degree in Social Work or Psychology with at least 1 year of counseling experience.

Experience: All candidates must have at least 2 years working in child welfare.

Credentials: Must have LPC license with in 1 year of employment, valid driver`s license, car insurance and a valid first aid and CPR card.

  3. First aid and CPR certified
  4. Passion to make a positive impact on the lives of children in the community and to educate them to proficiency
  5. Ability to observe participant behavior and participation, assess appropriateness of such, and enforce safety regulations and emergency procedures.
  6. Visual and auditory ability to identify and respond to environmental and other risks related to an activity.
  7. Physical endurance to engage children and adolescents in respective activities
  8. Physical strength and ability to respond appropriately to situations requiring medical intervention, and de-escalation.
  9. Cognitive ability to provide leadership to achieve participant goals and objectives.
  10. Excellent communication and organizational skills
  11. Able to work collaboratively with teams
  12. Demonstrated reliability regarding attendance and work performance
  13. Ability to be creative, flexible and adaptive in ALL activities and responsibilities
  14. Must be a team player
  15. Must want to help address the needs of the community
