SUMMARY OF DUTIES

An On-Call Wellness Clinician/Coach's responsibility includes performing health screening and coach activities and assisting our clients in finding new ways to inspire healthier habits. The Wellness Coach works closely with our clients and employer groups in promoting health and wellness, thus helping individuals realize their personal best health. The Wellness Clinician/Coach is also responsible for developing a wellness coaching relationship with clients and assisting the client through the process of achieving better health through education, support, and encouragement. Coaching may be provided face-to-face, telephonically, virtually, and via secure email.

ESSENTIAL FUNCTIONS / PRIMARY RESPONSIBILITIES

  • Perform the initial screening assessment including but not limited to confirming informed consent, personal demographics, and personal medical history.
  • Take an accurate blood pressure reading.
  • Properly perform a finger stick blood test utilizing a capillary tube and CardioChek Plus machine.
  • Must be able to calculate BMI and accurately measure waist circumference.
  • Performs other assigned clinical procedures as needed, including HbA1c testing.
  • Assess the client's readiness to change.
  • Provide personalized coaching for clients with a focus on collaborative solutions, strategies, and client generated goals.
  • Demonstrate a holistic approach in clinical practice.
  • Increase accountability and responsibility in individuals to achieve personal health goals.
  • Encourage adoption of habits conducive to a higher quality of life.
  • Document a customized care plan for clients including defined goals, a plan of action, and follow-up.
  • Support health promotion, disease management and care management services as needed.
  • Provide care consistent with all relevant professional standards for on-site health screenings and education programs.
  • Accurately document all clinical data and coaching follow-up data as designated by HealthYou policy.
  • Coordinate care with other health clinicians and/or other health-related vendors and professionals, when indicated, for optimal care.

PROFESSIONAL REQUIREMENTS

  • Professional in appearance, language, and all interactions with staff and clients.
  • Excellent communication skills.
  • Works well in a team environment.
  • Business Casual attire when interacting with staff and clients.

POSITION REQUIREMENTS

  • Graduate of an accredited Health Care Program, Registered Nurse, Registered Dietitian, Exercise Physiologist - master's degree preferred but not required.
  • Current professional license, if applicable.
  • Health Coaching Certification required if professional licensure does not allow health screening/coaching across state lines.
  • Current HIPAA certification within 6 months of hire.
  • Current OSHA certification required within 6 months of hire.
  • Current CPR certification within 6 months of hire.
  • Exceptional in effective interpersonal and communication skills.
  • Knowledge and use of medical equipment; specifically, CardioChek Plus, and Afinion A1c.
  • Medical experience with fingerstick blood draw and vital signs.
  • Proficient with computerized medical records programs and computerized spreadsheets.
  • Excellent customer service skills.
  • Valid driver's license.

KNOWLEDGE, SKILLS & ABILITIES

  • Performs other related duties as assigned by the HealthYou Clinical Staff.
  • Follows HealthYou Clinical Protocols.
  • Communicates potential risk management issues per policy to HealthYou staff.
  • Submits all documentation immediately upon completion of client screening and coaching sessions.
  • Provides clinical and subjective observations about participants to the HealthYou staff.
  • Attends, at minimum of 75% of staff meetings and required training sessions.

PHYSICAL REQUIREMENTS & ENVIRONMENTAL CONDITIONS

  • Regular exposure to stresses.
  • The workload required to perform this job requires the ability to adapt to change.
  • Some portions (10 - 50%) of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity, e.g., typing, handwriting, or machine operations.
  • Duties involve some exertion of physical effort, typically lifting and transporting medical equipment and supplies.
  • Professional office setting, while in the office.
  • Work may require evening, night, or weekend assignments on occasion. Schedule varies according to business demands and needs.
  • The nature of duties performed presents minimal potential for job-related accidental injury. Responsible for adhering to all OSHA and Universal Precaution Standards associated with taking blood and disposing of materials used for taking blood.
  • Some portions of daily assignments involve ergonomic risk, such as regular repetitive tasks.
  • Responsible for adhering to all safety policies and procedures of HealthYou.
  • Some portions of assignments may require travel to client locations via car, train, air, subway, or bus.
