Responsibilities:
- Plan, develop, organize, implement, evaluate, and oversee the social services program to support resident needs.
- Assist in the creation, administration, and coordination of departmental policies and procedures.
- Develop and implement strategies to identify and address the medically related social and emotional needs of residents.
- Collaborate with interdisciplinary teams on discharge planning, social care plans, and resident assessments.
- Conduct interviews with residents and families in a private and compassionate manner.
- Complete administrative duties, including forms, reports, and documentation, and submit them to the Administrator as required.
- Actively involve residents and families in planning social service programs whenever possible.
Requirements:
- Prior experience in a Skilled Nursing Facility or similar healthcare setting is prefferred.
- Bachelor's degree in Social Work or a related field.
- Strong interpersonal, organizational, and communication skills.
- Demonstrated ability to work collaboratively in a team-oriented environment.
