Your Role

As an RNAC , you will play a key role in the resident assessment process, ensuring the accuracy and completeness of MDS documentation and compliance with regulatory standards.

Key Responsibilities

MDS Coordination : Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including RAPs and Triggers.

Policy Development : Maintain and periodically update written policies and procedures governing MDS and care plan development.

Regulatory Compliance : Participate in facility surveys conducted by authorized government agencies.

Resident Evaluation : Assess each resident’s condition and pertinent medical data to determine individualized needs.

Team Coordination : Work with the DON to inform all assessment team members of new admissions and ensure participation from external care team members, including family representatives.

Care Plan Development : Develop preliminary and comprehensive assessments of nursing needs, utilizing required documentation forms.

Team Training and Oversight : Ensure assessment team members understand the importance of accurate and complete documentation, emphasizing compliance and avoiding penalties.

Qualifications

RN License : Licensed Registered Nurse (RN) in the state of Pennsylvania (required).

Experience : Minimum of 2 years as an RNAC in a long-term care setting (required).

Technical Expertise : Knowledge of PDPM, MDS, managed care, and bundled payment processes (required).

Regulatory Knowledge : Extensive understanding of state and federal regulations, including the state survey process (required).

Skills : Excellent oral and written communication skills with strong computer literacy.

Leadership : Proven ability to provide training, oversight, and corrective actions.
