Job Title: Resident Care Coordinator
Location: Gardens Care Scottsdale, 9185 E Desert Cove Ave, Scottsdale, AZ
Reports To: Executive Director and Resident Care Director
Position Summary
The Resident Care Coordinator at Gardens Care Scottsdale is responsible for overseeing the care and services provided to residents in our assisted living and memory care communities. This role ensures that high-quality care is delivered with compassion, aligns with regulatory standards, and meets the personalized needs of each resident. The Resident Care Coordinator will work closely with staff, families, and residents to promote an environment that fosters dignity, independence, and well-being.
Key Responsibilities:
- Care Management:
- Coordinate and monitor individualized care plans for residents based on their physical, mental, and emotional needs.
- Regularly assess residents' health conditions and collaborate with medical professionals to ensure appropriate care is provided.
- Oversee the medication administration process and ensure that residents' medication needs are met accurately and on time.
- Communication:
- Serve as the primary point of contact for residents and their families regarding care updates, concerns, and any changes to care plans.
- Foster open communication between residents, families, caregivers, and healthcare providers.
- Conduct family care conferences and resolve any resident or family concerns promptly and professionally.
- Staff Leadership:
- Lead, train, and mentor caregiving staff to ensure adherence to Gardens Care's standards of resident care.
- Monitor staffing levels to ensure adequate coverage and support to meet the needs of residents.
- Participate in the hiring and evaluation of caregiving staff, ensuring that all staff are well-equipped to provide exceptional care.
- Regulatory Compliance:
- Ensure compliance with state and federal regulations regarding resident care, documentation, and record-keeping.
- Maintain accurate and timely resident documentation, including care plans, medication records, and incident reports.
- Stay up-to-date with current senior living industry regulations and implement any necessary changes to maintain compliance.
- Resident Wellness:
- Promote and support resident engagement in wellness programs, activities, and events to enhance their quality of life.
- Monitor changes in residents' conditions and respond proactively to any health or behavior changes.
- Collaborate with the Activities Coordinator to ensure that programming meets the physical and cognitive needs of residents.
Qualifications:
- Education: Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred. LPN or RN certification is highly desirable.
- Experience:
- Minimum of 2 years of experience in assisted living, memory care, or skilled nursing environments.
- Experience with care coordination, staff supervision, and regulatory compliance in a senior living setting.
- Skills:
- Strong leadership and team management abilities.
- Excellent interpersonal and communication skills.
- Compassionate and patient-centered approach to care.
- Proficiency with electronic medical records (eMAR) and Google suites.
Working Conditions:
- Full-time position working Sunday through Thursday
- Will be scheduled to work two shifts each week to effectively know all service plans and resident needs
- Must be available for occasional on-call responsibilities, including weekends and evenings, to address resident needs.
- Physically able to assist with resident care when necessary, including bending, lifting, and mobility assistance.
Job Type: Full-time
Pay: $55,000.00 - $65,000.00 per year
