What are the responsibilities and job description for the Residential Rehabilitation Team Leader position at Garrett County Lighthouse, Inc.?
Job Duties:
1. Updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email; c) listening and responding to voicemail; d) reading Anchor update report; e) reading all required updates including the team bulletin board; f) reviewing Special Plan information to get a working knowledge of the current situation and possible interventions needed; g) obtaining assigned agency cell phone.
2. Establishes a professional, caring relationship with the people served by the team. Has awareness of and is sensitive to each person's (and of co-workers’) cultural, racial, and ethnic background. Oversees and provides as necessary, all direct services as required by the residents’ IRP services, including medication monitoring/med charts, support with activities of daily living skills, entitlement coordination, etc. Provides services required by RRP regulations and covers shifts as necessary. Ensures operations, which allow the program to pass all planned and unplanned inspections and audits.
3. Monitors the Residential Rehab Program ensuring all service targets and rehab outcomes are met; focuses team attention on resident satisfaction issues. Conducts quality assurance by reviewing all Individual Rehab Plans (IRPs), ensuring that they meet agency and regulatory standards. Completes and enters reauthorizations for services in a timely manner and follows-up through the appeal process when warranted. Provides guidance and oversight to HSWs on quality assurance and improvement planning on the Individual Rehabilitation Plans (IRP) of the people receiving services; ensuring planning and documentation are in accordance with regulatory standards (including obtaining of required Health Status Reports) and meet the needs of the individual being served. Follows-up with Critical Incident Reports when written.
4. Oversees all resident services including medication monitoring, support with activities of daily living skills, entitlement coordination, etc. Provides services required by RRP regulations. Ensures operations, which allow the program to pass all planned and unplanned inspections and audits.
5. Supervises all Residential Rehabilitation staff; schedules staff to ensure minimum staffing standards are maintained; completes routine evaluations for staff.
6. Ensures the houses meet fire safety and health and safety standards; addresses any concerns in these areas immediately and adequately; ensures that the houses are well organized and clean at all times. Oversees completion of annual drills in the residential homes/apartments, as required by CARF.
7. Develops and maintains professional relationships with other agencies and organizations relevant to the RRP and works cooperatively with them. Completes monthly reports including, BHA report. 24-hour Team Leader also completes monthly Environmental Report. Oversees the Residential Rehab Program budget; ensures all purchases are needed and follows established agency fiscal procedures; ensures budget targets are hit.
8. Supervises all areas of entitlement management and coordination including Social Security, Medicaid, SNAP benefits, etc. Provides technical assistance to staff to ensure all available benefits are accessed; when necessary, serves as a liaison with other agencies providing the benefits.
9. Participates as a member of the leadership team. Attends meetings as required, including Update Meetings, Leadership Meetings, staff recognition events and full staff meetings. Focuses on meeting all COMAR, CARF, HIPAA and other regulatory requirements and encourages other staff to do the same. Updates and manages relevant Policies and Procedures and other related documents. Prepares evidence for CARF audits. Attends certification/recertification training as scheduled. Completes all other training as scheduled/assigned. Completes necessary train-the-trainer training in order to train agency staff (such as DLA-20 Training).
10. Performs other tasks related to the overall scope of the job as assigned.
