Description

Examples of Duties

The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.

• Enrolls and maintains a caseload of 25 - 28 clients, providing bi-weekly visits of 1 to 1 1/2 hour in duration from early pregnancy until the child's second birthday. Develop and maintain therapeutic relationships with clients utilizing reflection and motivational interviewing.
• Provides in-home visits to vulnerable, first time pregnant women and their families who are eligible for Nurse-Family Partnership (NFP), in accordance to the NFP Model Fidelity (19 model elements) and guidelines.
• Assess physical, emotional, social and environmental needs of women and their families as they relate to the six NFP domains, following nursing process implementing NFP into nursing practice and formulates a nursing diagnosis based upon assessment and client goals.
• Assist with goal setting and attaining outcomes by providing education, support and referral resources. Evaluate client progress toward stated goals and outcomes and plan home visits in accordance with client goals.
• Maintains clear, effective, open, honest communication with internal and external customers and colleagues while representing NFP's vision, mission and model in actions, which promotes a positive work environment to promote productivity, mentoring, teamwork and cooperation.
• Consults and collaborates with other professionals involved in providing services to women and their families. Participate in community outreach to maintain relationships with community and referring partners.
• Participates in quality improvement activities, maintaining confidentiality and appropriate documentation. Must be able to role model positive behaviors and teamwork. Serve as a preceptor to new PH Nurse Care Managers (PHNCM) and/or student nurses as needed.
• PHNCM Safety: Willing to provide home visits in a variety of neighborhoods. Visits should be between 8:00 am and 5:00 pm, however occasionally may need to flex hours to accommodate client schedule. Be familiar with and compliant with NFP Safety Policy.
  
  

Minimum Qualifications

Graduation from an accredited college or university with a Bachelor's degree in Nursing with two (2) years of nursing experience is required. Must be currently licensed by the State of North Carolina as an RN in good standing with a valid BCLS CPR certification and drivers license.

A copy of your NC RN License and CPR certification must be attached to your application. Failure to attach these documents will result in your application being rejected.
