Client Coordinator
Overview:
Gateway Center of Monterey County is dedicated to empowering adults with Developmental Disabilities to lead fulfilling lives with dignity and independence within the community. As a Client Coordinator, you will play a crucial role in supporting individuals with Developmental Disabilities to achieve their goals and enhance their quality of life.
PRINCIPLE DUTIES:
The client care coordinator is a member of the interdisciplinary team and therefore is an integral part of all client care processes and activities of the facility. The client care coordinator works closely with the RN, the QIDP, the attending physician, and the direct care staff to assure the client is receiving optimal health care and outside services. The client care coordinator works under the supervision and direction of the RN Consultant in coordination with the QIDP.
The client care coordinator is responsible for the following:
CLIENT HEALTH SERVICES:
· In conjunction with the RN, QIDP, and MD, schedule and transport clients to routine and emergency medical and dental appointments, including labs and specialty clinics.
o Accompany clients to all appointments and serve as their advocate, i.e. communicate symptoms, capabilities, limitations, etc. to MD, as well as ICF/licensing requirements that may affect new orders. Ensure that orders and recommendations are in line with ICF and client needs
o Assist QIDP with the communicate medical appointment updates with larger ID team and facility MD. Ensure that new orders are implemented program-wide, including providing necessary staff trainings and communicating with day program (e.g. in-servicing staff on new diet orders, purchasing necessary supplies, ordering medications, etc)
· Provide oversight to direct care staff regarding medication administration and medication administration goals.
o Monitor all medications to ensure they are being passed accurately, and recommend additional staff training when appropriate
Participate in client assessments as directed by the RN.
o Under supervision of RN, create and implement temporary care plans based on MD orders and RN assessment. Schedule and monitor biweekly weight and vitals monitoring for all clients; provide trainings to DSP staff regarding vitals monitoring and reporting.
· Keep a log of all appointments and schedule follow up visits based upon MD recommendations and RN requests.
· Ensure that all durable medical equipment is maintained in optimal working condition
o Including assistive devices (CPAPs, AFOs, glasses, hearing aids, glucometer, etc)- maintain a regular cleaning schedule for all equipment and train staff on how to use assistive devices/equipment
· Assist with recommendations and implementation of Pharmacy medication review regimen
o Including ensuring all scheduled, overstock, and OTC medications are properly labeled and stored, purchasing necessary equipment, maintaining current drug handbook, updating emergency MARs/TARS monthly, etc
· Assist with medication ordering and monitoring medications under the guidance of the RN
o Facilitate all medication ordering and deliveries, including monthly cycle fill medications. Check all deliveries for accuracy. Communicate with pharmacy regularly regarding scheduling new orders, medication deliveries, emergency medication supplies (E-kit and back-up pharmacy), etc. Order and maintain appropriate stock of over-the-counter medications and medical supplies- keep track of inventory and expiration dates
· Maintain a weekly appointment calendar
o Ensure all clients are up to date on necessary appointments and schedule as needed
· Ensure that all new orders are faxed to the pharmacy in a timely manner.
· Conduct assessments of daily living skills under the direction of the RN Consultant
· Administer all morning medications, topical treatments, and medication objectives for all clients M-F
· Order regular labs for all clients and schedule lab draws/urine tests. Keep track of lab orders from different MDs to be combined into single lab draw when possible. Monitor lab results and notify RN/MD with abnormal lab results
· Assist QIDP with the EMDs, MARs, Physician’s Orders, and Qmar up to date to reflect new orders and diagnoses. Keep all documentation and charts updated with current information. Maintain policies for maintenance of medical supplies, or create new policies for new devices
· Monitor bowel health for all clients and notify RN with any issues
· Attend monthly facility MD visits; compile paperwork, issues, lab orders etc for MD’s attention, signatures, etc. Advocate for residents
· Ensure medication objectives determined at IDT meetings are being implemented and documented accurately
· Work in Therap with all paperwork charted by RN.
· Coordinate and pack medications for out of town visits
Participate as a member of the interdisciplinary team and is responsible for:
· Attend and participate in ID Team meetings
· Ensure that ordered medical equipment is available and is being used; recommend to RN possible needed equipment
· Serve as resident advocate
· Give monthly presentations at staff-wide meetings updating staff on clients’ medical status, diets, assistive devices, and any other medical or program issues
SCHOOL/DAY PROGRAM/OUTSIDE SERVICES:
· Communicate upcoming scheduled appointments to all outside programs
· Communicate with day programs any medical updates, changes in conditions, and medication changes
FACILITY SERVICES:
· Shares on call duties with QIDP/RSC
· Responds to emergencies and emergency preparedness as required
· All other duties as assigned by the QIDP/Administrator
· Participate in annual and non-scheduled licensing visits under direct supervision of DPH- answer licensing questions, pass meds under observation, etc
REQUIREMENTS:
· Valid CA driver’s license and clean driving record
· Fingerprint clearance through the Department of Justice
· Must have thorough knowledge of developmental disabilities, quality program services, administrative and budgeting practices, and applicable Federal and State Regulations
· Must have the ability to establish and maintain effective working relationships with individuals supported their families, the QIDP/Administrator, RN, DSP, Consultants, and governmental agencies. The ability to develop positive community supports.
· Applicants must have two years experience working with individuals with developmental disabilities
· Current CPR/First Aid.
· Complete and pass a TB test/COVID-19 test and health screening exam
· Flu shot and COVID-19 vaccine
