POSITION: SUPPORT and ENGAGEMENT SPECIALIST – Project Independence
POSITION OBJECTIVES
The Support and Engagement Specialist provides complex and varied assignments including transportation and supervision, data collection, evaluation of information, report development, tracking of assignments, and other related duties. Assists with transportation needs in including picking up medications, transporting Job Interviews, Trainings, Job Fairs, Community College, Trade Schools and Community Partners etc. Scheduling and transporting to appointments. Transport to interviews, court, visitation, GED assessments, and other medical appointments. The Support and Engagement Specialist will provide direct services to patients to strengthen and promote self-sufficiency, and help patients achieve their goals. Maintain vehicle logs and records for the current vehicles assigned to the Program.
ESSENTIAL FUNCTIONS
1. Thoroughly understand the clinical services and the various programs offered at Gateway and in the local community.
2. Know the goals of Gateway’s services, including understanding the concepts of recovery and wellness for patients experiencing substance use, mental health, and patients.
3. Assist the participants of Project Independence by providing services to support their individual goals.
4. Assist Project Independence participants who are seeking medical, employment, education, housing, and services.
5. Complete data collection activities as required by Project Independence.
6. Conduct holistic assessment of the patient’s needs and goals to identify in-house and community based services that will benefit the patient.
7. Document all services provided in a complete, timely, and accurate manner.
8. Fostering collaboration between Gateway and community stakeholders to support effective service delivery.
9. Advocate for patients to receive services and resources necessary to assist them, including services needed when they discharge from the program.
10. Support patients in the transition from Alumni Housing services to the community.
11. Provide warm hand-offs to service providers whenever possible.
12. Adhere to guidance on case management best practices, including understanding the core competencies for case management
13. Maintains current, accurate information in each record to include all activities, contacts, and communications. Obtains consent and releases of information when indicated.
14. Act as a multidisciplinary and collaborative team member with other healthcare professionals to assist in providing the best care possible for the patient.
15. Establish a consistent and open relationship with all Gateway programs and staff.
16. Provide excellent customer service to patients, their natural supports, other service providers, Gateway team members, community members, the managing entity, and any other person encountered in a professional context.
17. Provide regular written updates to the managing entity regarding patient engagement, enrollment, and progress.
18. Participate in scheduled meetings, including case consultation/staffing, staff meetings, and collaborative meetings with other providers.
19. Understand the various resources available in our community to address patient needs.
20. Must be willing to meet patients in the community at hospitals, clinics, shelters, jails, mental health /addiction programs and/or facilities.
21. Other duties as assigned.
EMPLOYMENT STANDARDS
1. Must be eligible through DCF and AHCA with Level II Background checks throughout employment.
2. Must abide by the organization’s Code of Conduct, policy and procedures, and state certification code of ethics.

QUALIFICATIONS
EDUCATION/EXPERIENCE/CERTIFICATION /LICENSURE
· Bachelor Degree preferred
· High School Diploma or GED required
· Valid Florida driver’s license and the ability to pass the organization’s driving test and meet standards for the organization’s insurance.
· Experience in social services/substance abuse programs preferred

· Ability to interact in a professional manner with local community, referral sources and patients

KNOWLEDGE
· Understanding of the 12 step recovery programs, office procedures, customer service, organizational policies and procedures, telephone and email etiquette, knowledge of Title VI, EEO, and Discharge Procedures.
