What are the responsibilities and job description for the BHHO Health Home Coordinator position at Gateway Community Services LLC?
The Nurse Care Manager shall provide primary care consultation, and psychiatric care consultation, and work with the Behavioral Health Home, the Health Home Coordinator, and the member to provide other Section 92 services as necessary. This role will be providing light nursing consultation and care in the clients' homes (e.g., explaining procedures, stocking pill containers, educating on appointments, and the like). The population is primarily refugees/immigrants struggling with aging and/or chronic health conditions; interpretation is provided. This position reports to the Clinical Director.
JOB TYPE: Part-time, 10 hours/week maximum
DUTIES AND RESPONSIBILITIES
- Support and closely coordinate mental health care with the patient’s primary care provider and, when appropriate, other mental health providers.
- Provide provider education about common mental health and substance abuse disorders and available treatment options
- Participate in regularly scheduled (usually weekly) caseload consultations with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s PCP.
- Consultations will focus on a new clients to treatment or who are not improving as expected.
REQUIREMENTS, one of the following:
- A registered nurse
- A psychiatric nurse licensed as a registered professional nurse by the state
- An advanced practice psychiatric
- A mental health registered nurse licensed as a nurse practitioner
- A clinical nurse specialist by the state
