Bay Area Home Health company seeking a compassionate RN in Alameda and neighboring counties.

Responsibilities:
- Provide patient-centered care and case management services to individuals in need, and work one on one with patient/ family to improve health and stability.
- Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and facilitate patient care and goals
- Monitor and evaluate patients' progress and adjust care plans as necessary
- Ensure accurate and timely documentation of patient information and medical records including clinical notes, orders, and charting within the Medicare required 48-hour window
- Advocate for patients' needs and rights within the healthcare system
- Educate patients and their families on disease management, treatment options, and available resources
Qualifications:
- Valid Registered Nurse (RN) license in California
- Bachelor's degree in Nursing (BSN) preferred
- Current CPR/ BLS Certification
- Valid Driver's License
- Customer service experience
- Minimum of 1 year experience with wound care.
- Strong knowledge of medical terminology, physiology, and diagnostic evaluation
- Experience in case management or managed care is highly desirable
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to work independently and make sound clinical judgments
- Proficient computer skills for documentation and data management
- Experience with Synergy EMR is preferred but not required
- Experience documenting accurate OASIS data is preferred but not required
- Understanding of the PDGM Medicare model
- Other duties as assigned
