Job description
Gateway Homes is seeking a part-time Care Coordinator for our supervised location in Alexandria, VA. This position is responsible for providing oversight and assistance to residential clinical services to residents.
The schedule is part-time Monday - Friday 8am - 4pm.
Duties and Responsibilities:
- Updating and managing the Case Management calendar appropriately with client appointments
- Assisting clients regarding the nature of medical/psychiatric appointments and educate the client regarding follow up actions
- Ensuring that new client’s prescriptions are sent to the pharmacy and get successfully into the CareSuite system
- Completing and organizing Medical Record Sheets for each client
- Presenting medical/psychiatric appointment outcomes during staff meetings
- Other duties as assigned
Minimum Knowledge, Skills, and Abilities:
- Knowledge of Microsoft Office including Word, Excel, and Access
- Good computer and data entry skills
- Ability to perform simple math and communicate effectively, both orally and in writing
- Ability to maintain effective working relationships with other employees
- Must be able to sit or stand for 8 hours and lift up to 20 pounds periodically
Required Experience or professional qualifications:
- College degree in human services field or prior training in mental health; prior experience in mental health in lieu of degree may be considered (Not Required)
- Training requirements as established by DBHDS licensing
The position is exempt from the Fair Labor Standards Act and is subject to the personnel policies of Gateway Homes, Inc. This position reports to the Team Leader.
