Gateway Hospice, a growing hospice agency is looking to fill the position of RN Case Manager for the Washington/South Hills area. This is a full-time salaried position working Monday - Friday 8:30 a.m. to 5:00 p.m. with minimal on-call duties. Brief Description:
Responsible to the Clinical Care Coordinator to provide nursing care and hospice case management to hospice patients and families. Implements nursing interventions and coordinates the Patient/Family Interdisciplinary Plan of Care. The RN Case Manager adheres to the regulatory process required by the State Board of Nursing and Federal Conditions of Participation.

- RN Nursing License in the State of Pennsylvania
- Valid Drivers License with acceptable driving record and a dependable vehicle.
- At least one year of nursing experience preferably in hospice or homecare. Nurses with Oncology, med Surg, Oncology experience also urged to apply.
- Involves daily travel to patients' home and facilities.
