Gateway Rehab Center (GRC) is seeking a passionate and dynamic APP to join our dedicated team! Our APP is responsible to provide high-quality patient care, collaborate with the healthcare team and contribute to our mission of helping all affected by addictive diseases to healthy in body, mind and spirit. In this role, you will travel to the various GRC locations on a regular basis.
This position does not supervise the work of others but is responsible for oversight of patient care management. This position receives monitoring and supervision of activities by the supervising physician.

Responsibilities
- Performs patient comprehensive patient assessments, including medical history and physical exam.
- Assesses, diagnoses, and treats coexisting medical and psychiatric disorders.
- Triages and manages emergencies.
- Develops and implements personalized treatment plans, including prescribing medications, therapies, etc.
- Collaborates with the health care team to coordinate patient care.
- Assess and record patient progress within the parameters of established protocols.
- Order and follow-up on lab tests and other diagnostic tests and / or procedures as appropriate.
- Review medical reports and results submitted by laboratories, hospitals, and physicians.
- Educate and counsel patients and families regarding disease management, preventative care, and healthy lifestyle choices.
- Maintain accurate and up-to-date medical record documentation in compliance with regulatory standards.
- Attend mandatory GRC in-services and trainings.
- Provide education and consultation with other members of the interdisciplinary team.
- Stay current with medical advancements, evidence-based practices, and professional development activities.
- Contribute to quality improvement initiatives and committee recommendations as needed.
- Orient new staff APP’s.
- Provide care via telehealth platform as needed.
- Perform other duties as needed.
