Home care provider ship is a family setting for adults who are unable to live alone due to a physical or developmental disability. Providers do not need to be married or own their home, they may rent, be single, or have a significant other they help to provide for. Providers should possess patience, common sense, a genuine acceptance of the person with the disability and the willingness to open their home to an adult with intellectual developmental disabilities. Be a part of a support team that assists the individuals to live, work, learn, and connect to the local community while working to increase independence in their daily life.
We are looking for a Home Care Provider in the Milford area to move in with a young man in his home. The ideal setting for this male individual would be one that is quiet, calm and relaxing. He is the individual in his home and is very content and comfortable in his surroundings. Providers who are patient and have experience with anxiety are important. Looking for a kind, outgoing person to be a roommate for a young man with developmental disabilities and autism. This individual enjoys being physically active by going to the gym, hiking outdoors, participating in music therapy and community activities such as bowling, food shopping, and delivering Meals on Wheels. He is a very engaging young man with a great sense of humor and excitement for life and is a vibrant participatory part of his household. He also attends a full-time day program Monday through Friday which will allow for a caregiver’s personal/down time. The ideal roommate should be willing and comfortable to provide ongoing encouragement and support to him, while he learns important life skills such as meal preparation, simple housekeeping, and food shopping, etc. Experience working with individuals, amidst challenging circumstances a must and a background/criminal records check will be completed.
A monthly stipend will be provided.
Responsibilities/Activities:
- Assist, instruct, and increase independence in everyday living skills which include, household chores, meal preparation, recreational activities, learning activities, physical fitness activities, individual's chosen inclusionary and community activities, and money management. Transportation to local community activities as needed.
- Monitor individuals for changes in general health, appearance, behavior, and patterns of activities; notify Guardian and medical professionals of any changes.
- Support individual with daily personal care and hygiene tasks, including assisting with shaving, brushing teeth, bathing/showering, hair care, and dressing appropriately.
- Follow the Individual Service Agreement (ISA) assist in goal work each day; and collect data as required. Home visits are also required (2-4 visits a year) along with the yearly Service Agreement meeting with the area agency- service coordinator, guardian, day staff, if applicable.
- Recognize and respond to all emergencies, implement existing protocols and emergency procedures as needed including but not limited to behavioral, fire, medical, evacuation, etc. Monthly fire drills are required and need to be recorded.
- Comply with all laws and regulation pertaining to adult foster care and provisions of NH Medicaid funded services. Will need to be med certified to be able to administer medications to the individual.
Minimum Requirements:
- High School Diploma or GED required
- Experience in the developmental disabilities system preferred, but not required. HCPs who are passionate about working with individuals with developmental disabilities and have equivalent education or desire to gain experience in the field will also be considered.
- Moderate computer skills.

A successful candidate will have a personal vehicle, proof of 100,000/300,000 liability auto insurance, and a safe driving record.
