Essential Duties And Responsibilities
- Assesses the nutritional status of adolescent, adult and/or geriatric patient through documentation in the medical record. Uses parameters such as anthropometric measurements, visual assessment and interpretation of laboratory values and interviewing patient and family as appropriate.
- Accurately assesses and calculates formulations for parenteral and enteral nutrition and makes adjustments based on patient’s clinical status.
- Develops and revises a nutrition care plan based on information from the patient, medical record, family and health care team members
- Communicates effectively with all healthcare and food service team members as well as patients and families through conferences, interdisciplinary team meetings, individual consultation and committee participation. This includes telephone, written, and electronic communication.
- Bachelor's degree from four-year college or university. Registered Dietitian-Nutritionist (Commission on Dietetic Registration). State of CT Certification. Maintenance of required PDP (Professional Development Portfolio) which specifies 75 CEU per 5 year period through Commission on Dietetic Registration. State Certification, as required.
