Geisinger Community Medicine is actively seeking an experienced Advanced Practice Provider to join our innovative team as an 'Inboxologist' at our family medicine clinics in central Pennsylvania. This unique opportunity allows you to proactively manage patient interactions and healthcare needs through effective inbox management. In this role, you will be responsible for evaluating patient messages, adjusting care plans, and working collaboratively with nursing and other staff to ensure comprehensive patient care.
Key Responsibilities :
- Assess and establish presumptive diagnoses as well as implement treatment plans tailored to patient needs.
- Accurately document patient interactions in the electronic medical record and provide timely referrals for diagnostic testing.
- Prescribe, adjust, or discontinue medications and therapies based on evidence-based practices.
- Collaborate with physicians to report any medical management issues that pose significant health risks to patients.
- Engage in careful and effective communication with patients and healthcare team members.
- Instill confidence in patients through advocacy and by offering safe, ethical care with comprehensive health education.
- Utilize clinical expertise to manage patient communications effectively, including virtual triage and follow-up procedures.
- Incorporate best practices to optimize in-basket management and streamline patient experiences.
Minimum Qualifications :
Education : Master's Degree in Nursing (Required)
Experience : Minimum of 3 years in a related healthcare role (Required)
Certifications : Certified Nurse Practitioner - American Academy of Nurse Practitioners; Licensed Registered Nurse (Pennsylvania) - RN State of Pennsylvania; Basic Life Support Certification - Default Issuing Body
