Job Description
The Registered Nurse Case Manager - Care Coordinator is responsible for coordinating patient care services across the continuum of care. This includes assessing patient needs, developing individualized care plans, and collaborating with healthcare teams to ensure seamless transitions of care.
- Assesses patient needs through thorough evaluations and develops individualized care plans that address physical, emotional and social needs.
- Collaborates with healthcare teams, including physicians, nurses, therapists and other specialists to ensure comprehensive care.
- Coordinates patient care services across the continuum of care, including hospitalization, rehabilitation and home health.
- Develops and implements strategies to improve patient outcomes, reduce readmissions and enhance patient satisfaction.
Required Skills and Qualifications
To be successful in this role, you will need:
- Three years of experience as a Registered Nurse, preferably in a case management or care coordination role.
- A minimum of a Bachelor's degree in Nursing, although a Master's degree is preferred.
- Current licensure as a Registered Nurse in the State of Pennsylvania.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
