Geisinger Care at Home team is seeking an experienced Nurse Practitioner or Physician Assistant (APP) for a community-based position, covering Lycoming county, Pennsylvania.
Job Duties :
MAJOR DUTIES AND RESPONSIBILITIES :
Geisinger Care at Home APP is a field-based patient direct care position performing health risk assessments, depression screening and addressing care gaps for Geisinger Health Plan Gold members in Lycoming county, Pennsylvania. Office location is Geisinger Health Plan Hughes Center, Danville, PA.
ADDITIONAL INFORMATION :
- Work is typically performed in the patient's home.
- Approximately 5-10% of the visits are virtual telemedicine.
- APP must reside in geographic area due to nature of position.
- Schedule can be 5 - 8-hour days or 4 - 10-hour days.
- There is no call, weekend, or holiday work.
