About the Role
The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Your responsibilities will include:
- Coordinating total plan of care with other healthcare professionals involved in care
- Helping to achieve and maintain continuity of patient care through effective communication and collaboration
- Making initial and comprehensive nursing evaluation visits to determine primary focus of care
- Developing and implementing individualized plans of care in accordance with state-specific guidelines
Requirements
To be successful in this role, you must possess:
- A current RN licensure in the state of practice
- Current CPR certification
- A valid driver's license, vehicle insurance, and access to reliable transportation
