Job Opportunity: We are seeking a RN Patient Care Manager to join our team. As a RN Patient Care Manager, you will be responsible for supervising and coordinating clinical services, providing nursing care to patients in their homes as prescribed by physicians, and ensuring compliance with applicable laws and regulations.
Responsibilities:
">- Supervise and coordinate clinical services
- Provide nursing care to patients in their homes as prescribed by physicians
- Ensure compliance with applicable laws and regulations
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders
Requirements:
">- Current RN licensure in state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
