About the Role
The Home Health Patient Care Manager oversees the supervision and coordination of clinical services, ensuring high-quality nursing care is provided to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
- Delivers clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with the healthcare team.
- Coordinates patient referrals, clinician assignments, and evaluates patient orders to ensure timely start of care visits.
- Ensures patient needs are continually assessed and care rendered is individualized to meet patient needs, appropriate, and reasonable, meeting home health eligibility criteria and physician orders.
Requirements
To be successful in this role, you must have:
- Current RN licensure in the state of practice
- Current CPR certification
- Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation
