As a Registered Nurse (RN) Case Manager, you will play a pivotal role in coordinating and managing clients’ intermittent services. This position requires the development, implementation, and evaluation of comprehensive care plans, fostering collaboration with an interdisciplinary team to deliver care that adheres to the highest professional standards.
Key Responsibilities
- Collaborate with patients and caregivers to complete and submit the patient plan of care and admission paperwork within 72 hours.
- Conduct ongoing patient assessments and document visit details accurately, ensuring alignment with physician orders and care plan adjustments based on assessment findings.
- Identify patient-centered goals and develop actionable plans, implementing necessary interventions.
- Facilitate regular case conferencing with team members to ensure all aspects of the care plan are prioritized and addressed.
- Continuously revise the care plan to reflect changes in patient status and progress towards goals.
- Communicate any changes in patient condition to physicians and team members through thorough case documentation.
- Audit clinical records for compliance with state and federal regulations, especially during recertification and discharge processes.
- Ensure effective discharge planning begins at the start of care through a multidisciplinary approach and appropriate documentation.
- Work with the Prior Authorization Nurse to assess the ongoing need for services based on accurate data assessments.
- Strong assessment and evaluation capabilities.
- Excellent communication and interpersonal skills.
- Proficiency in developing and implementing patient care plans.
- Familiarity with clinical documentation and compliance requirements.
- Ability to collaborate effectively within a team and demonstrate leadership when necessary.
- Degree from an Accredited School of Nursing (BSN preferred).
- Current Wisconsin RN license.
- 1-2 years of experience in Med/Surg or Home Care settings.
- Valid Wisconsin Driver’s License and reliable, insured vehicle.
