Key Responsibilities
- Assist patients with daily living activities, including bathing, dressing, and mobility support, while following the care plan.
- Review and understand care plans prior to providing services, ensuring patient needs are met and promptly addressing any concerns to the Case Manager.
- Engage with patients to explain the care procedures being administered and involve them in their care for better adherence and comfort.
- Complete mobile care tasks in compliance with agency policies, including accurate logging of mileage and care activities.
- Maintain a clean and safe home environment by performing routine household tasks such as cleaning, meal preparation, and laundry as outlined in the care plan.
- Collaborate effectively with patients, family members, and agency staff, ensuring open communication about patient progress and concerns.
- Strong communication skills to facilitate interaction with patients and their families.
