Key Responsibilities
- Provide direct assistance with personal care tasks, such as bathing, dressing, and mobility transfers.
- Review patient care plans prior to delivering services, ensuring alignment with physician’s orders.
- Communicate effectively with patients about the care being provided, fostering their involvement in the process.
- Complete mobile care tasks in accordance with established care plans, ensuring timely and accurate documentation.
- Collaborate with patients, families, and agency staff to address concerns and enhance the care experience.
- Maintain a clean and supportive home environment by executing home maintenance tasks as specified in care plans.
- Strong communication abilities to engage patients and explain care processes.
- Effective time management skills to complete mobile care visits and maintain accurate records.
- Empathy and patience in providing assistance with daily living activities.
- Ability to work collaboratively with patients, their families, and the healthcare team.
- High School Diploma or equivalent required.
- Valid Wisconsin CNA certification allowing work in Federally Certified Home Health Agencies and Hospices.
- Previous experience in a public-facing role is essential.
- Current and valid Wisconsin Driver’s License, along with reliable and insured vehicle.
