We are seeking a compassionate and dedicated Home Health Aide for our Hospice program. In this pivotal role, you will provide essential support to patients in their daily lives, ensuring their comfort and well-being under the guidance of a Registered Nurse or Therapist, while adhering to physicians’ directives.
Key Responsibilities
- Assist with personal care activities, including bathing, dressing, and toileting under the supervision of the Case Manager.
- Review and understand the patient care plan before delivering care, addressing any issues with the Case Manager as needed.
- Engage with patients to explain the care being provided and involve them actively in their care process.
- Complete mobile care tasks according to agency policies and accurately document mileage logs.
- Work collaboratively with patients, family members, and agency staff, effectively communicating and responding to their needs.
- Perform regular home maintenance activities as outlined in the care plan, such as cleaning, meal preparation, and transportation.
- Strong interpersonal and communication skills to effectively engage with patients and their families.
- Ability to work collaboratively with a diverse group of individuals in a compassionate manner.
- Excellent organizational and time management abilities.
- High School Diploma or equivalent is required.
- Valid Wisconsin CNA certification that permits work in Federally Certified Home Health Agencies and Hospices.
- Previous experience in public-facing roles is essential.
- Must possess a current and valid Wisconsin Driver’s License along with reliable and insured transportation.
