Your role is critical in fostering a nurturing and health-oriented environment for our patients, aligning with physician directives.
Key Responsibilities
- Assist with the daily living activities of patients, including bathing, dressing, and mobility transfers, under the supervision of the Case Manager.
- Engage with patients to explain the care being provided and actively involve them in their treatment plans.
- Conduct thorough mobile care tasks upon entering and exiting patients’ homes, following agency policy.
- Communicate promptly with the Case Manager regarding any issues, concerns, or necessary revisions to patient care plans.
- Perform home maintenance tasks as outlined in the care plan, such as cleaning, laundry, and meal preparation.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Capability to assist patients with daily living activities and adhere to care plans diligently.
- Strong organizational skills for managing various home maintenance tasks.
- Collaborative mindset to work effectively with care providers and family members.
- A minimum of a High School Diploma or equivalent is required.
- Current Wisconsin CNA certification enabling work in Federally Certified Home Health Agencies and Hospices.
- Prior experience in public-facing roles is necessary.
- A valid Wisconsin Driver’s License and access to a reliable, insured vehicle are required.
