Key Responsibilities
- Provide personalized assistance with daily living tasks such as bathing, dressing, transfers, and toileting.
- Review and follow care plans prior to delivering care and communicate any concerns to the Case Manager.
- Engage with patients to explain the care being provided and encourage their involvement in the process.
- Complete mobile care tasks and maintain accurate logs in accordance with agency policy.
- Collaborate effectively with patients, families, and care providers to address needs and report relevant information.
- Execute home maintenance activities as outlined in the care plan.
- A compassionate and patient-centered approach to care.
- Strong communication abilities when interacting with patients, families, and team members.
- Teamwork and collaboration skills to work effectively with various stakeholders.
- Basic knowledge of healthcare practices and protocols.
- High School Diploma or equivalent.
- Valid Wisconsin CNA certification for employment in Federally Certified Home Health Agencies and Hospices.
- Previous experience working with the public is required.
- Current and valid WI Driver's License with reliable, insured transportation.
