- Conduct thorough assessments of patient conditions during scheduled visits, encompassing physiological, psychosocial, and spiritual considerations.

- Offer comprehensive education and support to patients and their families, fostering an understanding of care processes.
- Accurately document all patient interactions on the same day care was delivered, ensuring clarity and legibility.
- Communicate any changes in patient conditions or care plans to physicians, managers, and interdisciplinary team members swiftly and accurately.
- Strong assessment capabilities for diverse patient conditions.
