Job Summary

Key Responsibilities
- Assess and establish presumptive diagnoses for patients, implementing treatment plans and providing follow-up care as needed.
- Make accurate and complete entries into the electronic medical record, ordering diagnostic testing and prescribing medication as necessary.
- Evaluate and report any circumstances or approaches to medical management that present a significant risk to patient health or well-being.
- Communicate effectively with collaborative physicians, patients, PCPs, and referring staff through telephone or other means of correspondence.
