We are seeking a highly motivated Addiction Medicine Advanced Practitioner to join our team in Scranton, Pennsylvania. This is an exciting opportunity for a skilled nurse practitioner to contribute to the delivery of integrated mental health and substance use disorder services.
The successful candidate will connect and manage patients receiving Medications for Opioid Use Disorder (MOUD) and provide behavioral health counseling and social support to retain patients in care.
Responsibilities
- Participate in MOUD in community-based settings and receive additional clinical training beyond the didactic component.
- Develop and implement individualized treatment plans that address the unique needs of each patient.
- Provide education and counseling to patients covering health status, test results, disease processes, and discharge planning.
