Job Summary:
The Geisinger Care at Home team is seeking an experienced Nurse Practitioner for a community-based position, covering Luzerne and Lackawanna counties.
Job Duties:
- The Geisinger Care at Home Advanced Practice Provider is a field-based patient direct care position performing health risk assessments, depression screening and addressing care gaps for Geisinger Health Plan Gold members in Luzerne and Lackawanna counties.
Additional Information:
- Work is typically performed in the patient's home.
- Approximately 5-10% of the visits are virtual telemedicine.
- APP must reside in geographic area due to nature of position.
- Schedule can be 5 - 8-hour days or 4 - 10-hour days.
- There is no call, weekend, or holiday work.
Advanced Practitioners Receive:
- $10,000 Recruitment Forgivable Loan - a two-year work commitment.
- Comprehensive medical/retirement benefits starting the first day of employment.
- CME funds ($3,000/year) and paid 10 CME days/year.
- Company vehicle, computer, computer monitor, and cell phone are provided.
Experience Requirements:
- A minimum of two years of Advanced Practice Provider experience is required.
