Job Description
This full-time position requires at least three years of RN work experience. As an RN Case Manager in Pulmonary, your primary responsibility will be to assess, plan, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient or member's health status.
You will also be responsible for integrating evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the identified population.
