Job Summary
As a member of a multidisciplinary Palliative Care team, you will serve as a liaison with hospitals and other community organizations. Your responsibilities will include facilitating family meetings, addressing anticipatory grief and difficult family dynamics, exploring placement options, and providing access to social, financial, and legal services.Key Responsibilities
- Provide outpatient psycho-social evaluations for patients with advanced illnesses.
- Develop intervention plans, managing caseloads that may consist of inpatients, outpatients, and family members.
- Document all interactions in the electronic health record.
- Maintain effective communication with interdisciplinary teams throughout the care continuum.
- Participate actively as a team member, demonstrating a professional working relationship with all levels of staff.
- Liaise between medical and nursing staff, patients, and outside agencies.
- Address the complex needs of patients with advanced illnesses.
- Engage in in-service and staff development activities, including attendance, committee participation, and educational program suggestions.
- Pursue knowledge of palliative care benefits for patients and families, attending conferences, and seeking certification in palliative care social work.
- Communicate effectively using good interpersonal skills.
- Support internal and external customers.
- Contribute to performance improvement activities.
- Accurately code services using current billing codes according to system and department procedures.
The work environment is typically clinical and office-based. You will be accountable for satisfying all job-specific obligations and adhering to organization policies and procedures.
